1. Fill Out the Secured Online Intake Form or Send Us a Service Request Document

2. Verify the Information

You can choose to fill out the paper documents or online forms.

As soon as we receive your service request (parents or guardians), we begin the authorization process with your benefit source.

When we receive the authorization from your benefit source, we will set up the assessment visit for your child. (Remote Service Available)

After the assessment visit, our clinical director will develop an individualized ABA service program for your child.
